This patient vividly illustrates why home is important to the elderly. This patient requires frequent interventions and reassurance from the home care nurse, but is able to stay at home comfortably and “in her own environment.”

VNA Community Care Services - Lancaster, Pennsylvania

The value of telemonitoring in keeping patients at home is shown in this video. Home telemonitoring allows the clinicians to access early changes in the patient status. This patient describes how frequent and early interventions, as a result of the telemonitoring, permitted him to stay at home. This video also illustrates how the strong relationship between the home care nurse and patient provides a better opportunity to encourage patient self-management, understanding of disease and compliance with medications.

Davis Home Plus - Elkins, West Virginia

This video demonstrates the importance of telemonitoring to a patient in one of the 12 rural counties that Davis Home Plus serves. Since the telemonitoring system records her vitals daily, she is able to stay at home and take care of her husband. The close relationship between patient and nurse is also explained in detail.
