CALL ME FIRST Campaign documents

Looking for the best practice packet from past that has .doc templates for paper files/phone/magnets etc.. using the CALL ME FIRST info that is able to be modified with logos..numbers etc.. Ones I have found are .PDF and cannot seem to customize the document tools.

How have you been introducing the Best Practice tools in your agencies? Any ideas on how to get it to the clinician level and get clinician buy in?

Agency Parameters

What are the parameters that your agency is using as standard reporting parameters? I understand to answer NA to M2250a both sentences must apply ... MD has no patient specific parameters AND agency will use their standard reporting guidelines. What ranges have you identified?

Best Practice Tools to Plans of Care

Can someone share on how you incorporate best practices tools to plan of care, theres a lot of information to include. im not sure if its acceptable to write CHF protocol and or fall prevention protocol? Any ideas. Thank a lot
