Internal Chart Audits

What tool is your agency using for auditing skilled home health and hospice charts? With the advent of Oasis C and our recent imlplementation of Encore Delta Health software, we have nothing compatible to use. Any assistance is greatly appreciated!

Zone Tools, PEP

-our patients, direct care frontline clinicians, and case managers use the condition specific zone tools and my emergency plan for 2010- - in addition the depression zone tool, possibly appropriate for chronically ill patients with multiple cormorbidities - -my emergency plan is renamed to my medical emergency plan to make the distinction from the general safety emergency plan that is a part of the home health admission=

Home Health Compare

As we move closer to pay for performance, I just have to question the reliability of agencies reporting correct data. I recently looked at HH Compare stats for our state, and viewed the top performer. Interesting that that agency is able to post a 100% for patient's who's bladder contro improves. Amazing especially when you consider the state avg is 54%. Does anybody else question these???

Face to Face Encounter

While our agency is pretty luck to have the majority of our patients followed by a family practice group that does actually see their patients, I'm still wondering how this new proposal will work. If the MD's visit in the hospital doesn't count, and they might not see them for 2 weeks, then we must wait that long to have the Plan of Care signed??? Why is it okay to have an ecounter via telehealth but not in the hospital?? Can anyone explain the logic behind this proposed ruling??

Timely Initian of care

Our agency uses the date of referral to our agency to answer M0104. This date does not reflect the negotiated SOC date. Our HHC scores are low due to this. Are we answering this question correctly?
