Agency Comments

HHQI Team Remarks

Have not reviewed the Teletriage information yet. We do have a Telephone triage system in place but it could use enhancing.

Many agencies will have some of the best practices implemented already. Using the BPIP to enhance processes and interventions is an excellent plan.

I believe that we already practice teletriage but was not aware that we are.  There are some interesting aspects to this package that we can easily adopt and develop into a more formal practice.

Many in home health have not recognized teletriage as something they practice. Looking for resources in the package that you may easily adopt is appropriate.

I find the packages to be very informative and they have assisted us to enhance and improve the current quality improvement strategies. We do feel there is a lot of material to review in a very short period of time to try to implement with the staff. I found this package in particular to have great documentation tools which we would like to implement.

I agree — there is a lot of material to review. Use the ‘Pick and Choose’ method to best fit your agency needs (page 8 in the Teletriage BPIP). Glad you found the documentation tools helpful!

I loved this package. I have listened to phone calls to staff and supervisors and wished for such a tool or the time to write one, now here it is! Thank you!

The sample Teletriage Reference and Documentation tools are turnkey tools — ready for your staff to use. Please go to the Home Telehealth Reference 2006/2007 at www.medqic.org for more Teletriage reference and documentation tools.

It is all very good information...but actually too much.  It takes time to review, and then determine a plan for what to implement.  Before I finish one packet another or even 2 have arrived.  It is too much for the staff to do since their schedules are so full...so, even though we can pick and choose what we want to use, it seems a bit overwhelming.

You may want to plan which packages are more pertinent for you at the current time. You may want to only use the Fast Track Package some months, or even skip a month or two. You can always download the package to use at a later time. There is a lot of information in the packages, but our goal is to help home health agencies with resources. We certainly don’t want any to be overwhelmed. You may consider calling your state QIO for guidance in selecting which packages are more of a priority for your agency.

The Fast Track Guide is perfect for our small agency. We love it.

The Fast Track Package contains one of the package tools and some highlighted information. Glad you find it helpful!

We have not been receiving the Best Practices Packages.  We would love to be receiving these.  Not sure why we are not.

All the packages to date (Hospitalization Risk Assessment, Emergency Care Planning, Medication Management and the Home Telehealth Simply Summer Series) are available for download at www.homehealthquality.org

Our managers have requested copies of all of the teletriage reference tools. We plan to review them with our staff and decide which ones are of the most benefit to us. We will provide more staff education on our teletriage protocols, then we are going to copy them onto card stock, put them on a ring and provide all RN's with these tools.

This is a great idea. Thanks for sharing!

We love the clinical tracks and non-clinical tracks that can be placed on CDs and listened to as we drive to visits.

Good use of clinician’s time on the road. Thanks for sharing!

We are working on Case Management initiatives currently and want to re-visit the Teletriage Packet after this initiative in September.

Sure—each agency needs to use the packages that support their work. The current package may be one you download and save for future projects.

We have implemented the front loading of visits and phone calls using some information from that package. The teletriage information will simply add guidance and information for those nurses accepting calls. All the packages whether fully implemented or not have such a vast amount of information to supplement our current practices.

Thanks for this comment. It sounds like you are using some packages more in depth and are more selective with others. This is exactly what we want agencies to do.

Your tools are GREAT!!

We have improved our rate of Acute Care Hospitalization from 47.4 to 37%. Thank you for the tools to provide the highest level of quality care!

Wow! Great work by your agency!
