Everybody in the office, both skilled and non-skilled, is excited about the Emergency Care Planning.  By following this plan, adverse reaction can be justified or minimized.  This led to best outcome practice. Our consultant is also into the Best Practice Intervention! I am so glad that you are doing this and it is available to us!! Great idea looking at adverse events in relationship to emergency care planning!
Wealth of information! Very helpful having information separated into specific tracks. Thought Patient Emergency Care Plan example was wonderful. Thanks! The package is available as "Full Contents" or individual discipline "Tracks" - each agency can download what works best for it.
We have not been able to implement all things, but I like the way the material is set up. The fast track allows you to at least take minimal action until you can implement some of the other more involved actions. I really like the way you have set up different tracks for different disciplines. It is so difficult to reach all at a discipline specific level when doing education and these are great! Nice that the audio is given on a CD also; please continue this. (Only suggestion would be to have each audio on a different CD i.e. RN, therapy, MSW, aide each have own CD) Overall-great job! The materials are useful and realistic. Good idea about the Fast Track. It can give you a quick start with minimal action. It may also fit the needs of some agencies that want to "go easy" on certain topics due to having a good understanding of the intervention or just not enough time. The audio recordings are available as a download at www.homehealthquality.org and will be mailed on CD quarterly. Participating HHAs will receive April, May and June audio recordings in July. Currently, we have audio recordings for both clinicians and home health aides.
We currently have good emergent plans but will add from the material you have provided. Thanks! Certainly add anything from the tools that works with your current plans or tools. These are not copyrighted and are designed for each agency to implement as needed.
We are a small (two person) agency and cannot offer 24 hour on-call coverage, but we can improve our patient teaching utilizing more simplified best practice patient teaching outlines. Creative idea for a small home health agency to use the packages. Thanks!
These packages are very helpful, brief and to the point. I think the leadership review is especially helpful to call their attention to what is going on as I think that these areas get lost with all their other roles and meeting and reviewing with management in our agency has stirred up some commitment on their part. We are glad this is working to incorporate administration and leadership into the plan. We recognize the need to work with agency leaders and will continue to make this focus of the packages.
The package was easier fro me to use this month since I was familiar with the categories and what work last month. I'm looking forward to continuing to have these very helpful resources available each month as part of the education offerings I can provide for my staff. We are glad it was easier to use. The packages will continue to be in similar format-leadership, nurse, therapy, medical social work and home health aide tracks. There will be a "tool" to help agencies implement the package. The audio recordings and success stories will also be parts of the package.
The idea of having credit hours made the RNs complete the track. It would be good to offer more and for other disciplines. We are working on this!
This is simply too much information. We no sooner got through the March package when the April one was released!! We are a small agency with no dedicated QI staff; this method is very burdensome for us to implement. The fast track package is a step in the right direction; however, it is still too much. Yes - please use the Fast Track Package! It is important to understand that the packages are designed to meet the needs of many home health agencies (almost 5,000 as of 4/20/07!) Please feel free to modify and/or extract the most relevant information based on agency staffing issues and patient's needs. Agencies are not locked into utilizing the packages "as is." Integrate the information into existing processes and/or patient teaching to manage the patients better and/or maximize the patients'/caregivers' knowledge and ability to deal with their situation more independently.
