- The Home Telehealth Disease Management Series provides clinicians with a package of tools to improve the management of Heart Failure, COPD, Cancer, and Diabetes through the incorporation of telehealth technology. Each disease topic includes Patient Selection Criteria, Decision Support Tools, Patient Encounter Documentation Tools, Patient Self-Care Workbooks and Staff Education Guides. These tools may be easily adapted for the non-telehealth patient population as well. They are located on www.medqic.org.
