Who?
Best Practice Intervention Packages were designed for use by any home health agency to support reducing avoidable acute care hospitalizations. Any home care clinician can use the free educational materials.

What? 
Best Practice Intervention Packages
are a compilation of background information, leadership guidance, implementation tools and discipline specific education and application materials specific to the targeted best practice of the month.

Where?
All Best Practice Intervention Packages are downloadable from this Web site or on www.medqic.org under Home Health. You can click on any of the available packages in the right bar on this page or above in the schedule. Also, there is a link for all available free continuing educational units (CNEs) for RNs and for certificates of participation for therapists. See the Continuing Education Frequently Asked Questions (FAQ).  All continuing education will end on February 28, 2008.

Why?
The Best Practice Intervention Packages were designed to educate and create awareness of strategies or "best practice interventions" to reduce avoidable acute care hospitalizations.

How?
Home health agencies can pick and choose the components of the packages they want to use.

Too Busy?
Use the Fast Track option. The Fast Track package is ideal for the agency that finds they have limited time and resources in any given month and wants a "fast track" review of the tool and key points only.


Voluntary Surveys

Each month, a voluntary survey will be e-mailed to all campaign participants and posted to this Web site.  This survey will be available for approximately one week each month.  Findings from these monthly surveys will be posted to the Web.  Below are archives of previous voluntary survey findings. 


Agency of the Month

Every home health agency who submits a monthly survey within the designated time parameters will be eligible to be randomly selected as the HHQI National Campaign Agency of the Month.  This agency's information will appear in various promotional venues related to the campaign.  Below are archives of the selected Agencies of the Month:
