However, you are still encouraged to register for the campaign and will have full access to resources and materials in the meantime. For Home Health Agencies Home Health Agencies register through this link. Note you will need your CMS Certification Number (also known as a Medicare Provider number) to complete registration. Home health agencies register here to gain access to the Best Practice Intervention Package (BPIP) and other best practice tools. You will create a username and password here and use it each time you log in to the Web site.
NEW! For Other Healthcare ProvidersOther healthcare providers register through this link. Other healthcare settings can now register for the campaign and download educational tools including the BPIP. Use this link to initiate the registration process. Examples include: hospitals, nursing homes, skilled nursing units, rehabilitation units, health coaches, Quality Improvement Organizations, etc.
For Campaign Supporters
Campaign Supporters register through this link. You will receive a listing on this site and the ability to subscribe the campaign e-bulletin and list serve. Campaign supporters do not have access to healthcare provider educational materials. Examples include: home health or care transitions patients, families of patients, caretakers, friends of the campaign, etc.
