The Home Health Quality Improvement Summit occurred on January 11, 2007, at the Centers for Medicare & Medicaid Services in Baltimore, Md. “Preventing Avoidable Hospitalizations: Piecing the Puzzle Together” was the theme for this prestigious event that served as the kick-off to the national quality improvement campaign.
