“Community Hospital Home Health has a team of dedicated staff that promotes quality of care. The staff is all involved in monitoring and assessing the care provided by reviewing indicators that measure the quality they work so hard to deliver. One change our agency has made to help promote quality of care was to develop a Unit Practice Council where all of the staff meets to discuss quality of care, patient satisfaction, and other processes. The staff meets regularly to review indicators such as acute care hospitalization and oral medication management, and together they develop action plans that will facilitate even better quality of care. This team communication and empowerment to make positive changes has been a key factor in improving the quality of care our patients receive.
