At Rex Home Services we have recently implemented a new method to interface more effectively with our telehealth system and our primary supplier. We will eventually be able to interface with the hospitals, as well. Having interfaces with these components gives the agency staff a more effective and efficient means to provide coordinated care. Interfacing with our hospitals allows a timely and complete SBAR and medication reconciliation process. It also provides our patients and their caregivers increased peace of mind that our agency is working cohesively with their other healthcare providers.
As an agency we are very proud of our clinical staff and the quality of care they provide to our patients. We believe our most significant steps toward continuing to reduce ACH are the recruitment and retention of qualified clinicians who are invested in providing outstanding patient care, the close cooperation between home health, our hospitals and community physicians and the implementation of telehealth.
To this end we strive to provide ongoing, current in-house continuing education for the rehabilitation, nursing and aide staffs. Rex Home Services has had a 95% retention rate, excellent employee satisfaction survey results and most importantly, patient satisfaction results of >95% for the previous eight (8) years.
