We have utilized the exceptional tools, education, and resources that HHQI provides for best practices in order to achieve high level outcomes for our patients. Our philosophy is that we recognize the unique physical and emotional needs of each person receiving health care in the home. Phoenix Home Care L.L.C. has been able to reduce hospitalizations, improve medication management, and increase patient’s satisfaction of their home care experience. This was achieved through immediate patient identification on admission by the use of the Hospitalization Risk Assessment, tailored to our patients at highest risk. We use a Patient’s Daily Diary that contains key questions that would indicate if there is a change in a patient’s condition. We then provide early RN/MD intervention to reduce any risk of complications. We implemented home telephony, front loading visits, and the use of the Call First System along with the Emergency Plan Tool as recommended by BPIP.
Phoenix Home Care L.L.C. delivers state of the art home health services. We engage in a wide range of continuing education, clinical education, and other programs for professionals and the public. HHQI is instrumental in their educational support to maintain higher standards of practice for quality patient outcomes. We are most proud of attaining and continuously improving our low rate of Acute Care Hospitalization. We remain driven to continue to reevaluate processes in the pursuit of excellence, and strive to be the leaders in our industry for home care.
