The Home Nursing Agency and Visiting Nursing Association is honored to be nationally recognized this month by the Home Health Quality Improvement National Campaign. A strong patient focus is reflected in our mission of “dedication to the highest quality of customer service delivered with a sense of warmth, kindness and individual pride”. Home Nursing Agency has as its trademark a diversified menu of healthcare services available within its 13 county footprint in central Pennsylvania.
Targeting medication management and compliance as a direct link to improving both acute care hospitalization and medication management scores has been an ongoing performance improvement indicator for the Agency. Evidence-based practice standards have been incorporated into clinician reference tools and patient education material and serves as the foundation to achieve excellence in clinical outcomes. OASIS accuracy at the start of care and the development of innovative educational scenarios to educate clinicians has also provided positive results. Home Health quality improvement is a cornerstone of the strategic initiatives within the organization, and leadership goals have been aligned to maintain the momentum toward achieving outcomes.
