Southeast Home Health is excited and honored to accept the National HHQI agency of the month award for June. Southeast Home Health is a division of Southeast Health, a not-for-profit, 2-time Magnet Award winning Hospital and Regional System of Care in Southeast Missouri.
Our team utilized an evidence-based approach, information from our QIO, HHQI national campaign and our own historical agency information to develop and implement an in-depth disease management program. This disease management program is the cornerstone for reducing our acute care hospitalizations. The program primarily consists of an emergency ACH risk tool, emergency care plans, disease specific education packets, telephonic management and disease specific pathways for care.
Our continuous dedication and use of this program has allowed our team to consistently rank among the top agencies in the nation in reducing acute care hospitalizations. This exemplifies Southeast Health’s mission statement: ‘Together we make a difference through our commitment to excellence in health care.’
