"Home Health Services of Venice strives to provide excellent care to the people of our community. Helping our patients stay at home to manage their health has always been a priority.
Our clinicians have developed excellent relationships with area physicians, and work with them to prevent hospital stays. Our office is staffed by RNs 7 days a week, and RNs are readily available 24 hours a day. We believe we can prevent hospitalizations by being available to our patients when they need us. We teach our patients to manage their health care needs independently, which we believe reduces emergent care visits.
The Best Practice Intervention Packages provided by HHQI have given us useful ideas and practices to help our patients. In this ever-changing health care environment, it is vital to remain current with practices. We are developing a new Heart Failure specialty team, and will utilize information from HHQI in the development of protocols."
