December HHQI Agency of the Month

Teaching is one of our most valuable skills. We teach as hard and as soon as we can to give our patients the ability to manage their disease. Home Health is temporary, this knowledge will last them a lifetime. We believe in education for our patients and our staff and encourage them to specialize in their field of interest so that we have a well-qualified pool to draw from in providing good quality care.
We have used many ideas and tools provided by our QIO and the HHQI National campaign. We use telehealth monitoring for ‘at risk’ patients. We use an SBAR communication format with our doctors. We use the ‘Call Me First’ motto with every visit. We use a ‘Safe Moves’ Falls Prevention program to reduce falls and improve mobility.
We believe in working as a team and within 7-10 days of admission each of our patients are reviewed by the active clinicians in a case conference setting to make sure they are getting all the services they need to get better. All of these things have helped us reduce patient hospitalizations and improve patient outcomes.
As an agency we are very proud of each member of our home health team that allows our qualified staff to provide outstanding patient care.
