McLean Home Care and Hospice in Simsbury, Connecticut thanks the HHQI National Campaign for naming McLean Agency of the Month. We are proud of achieving a low rate of Acute Care Hospitalizations (ACH) that places us in the top 90th percentile nationally. Our not-for-profit agency serves McLean Affiliates’ active retirement community and a nine-town area. Our agency is part of a full spectrum of McLean services that have earned a strong reputation for the highest quality compassionate care.
We thank the HHQI National Campaign for providing our team with best-practice, freely accessible ACH tools that enabled us to maintain our low acute hospitalization rate along with high patient satisfaction scores. Every one of our staff is committed to helping our patients live comfortably in their own homes, optimally managing both acute and chronic conditions, and avoiding unnecessary trips to the hospital.
In the past year, our team launched “Living with Heart Failure” and “Breathe Easy” programs that also incorporate HHQI evidence- based tools. Our heart failure and COPD protocols include telemonitoring services, and both programs are integrated across the full McLean service spectrum. We have recently enjoyed HHQI seminars and tools concerning Care Transitions. We share this information as participants in the Qualidigm “Communities of Care” initiative to reduce preventable heart failure readmissions in Connecticut. Our quality improvement team looks forward to HHQI’s continued support of our goal to achieve the best patient clinical outcomes in the nation.
