Integrating Preventive Cardiovascular Health Into Home Health: A Pilot Study
describes a pilot study conducted in conjunction with the launch of HHQI's cardiovascular health initiative. The project aligns with the national Million Hearts®
initiative to prevent a million heart attacks and strokes. The data for the study was drawn from HHQI's Home Health Cardiovascular Data Registry
(HHCDR) where agency's submit a few pieces of additional patient information related to one or more of the measures that focus on aspirin as appropriate, blood pressure screening, cholesterol management, and/or smoking cessation.
The study demonstrates that home healthcare agencies can contribute efforts with the national initiative to reduce heart attacks and strokes. The results supported the expansion of the HHCDR nationwide which is now open to all Medicare-certified home healthcare agencies.
