BFCC-QIO Info Required for New Home Health CoP

The new Home Health Conditions of Participation (CoP) go into effect January 13, 2018. One of the requirements is section § 484.50(c) where the Home Health Agency (HHA) is required to provide names, addresses, and telephone numbers for a variety of community services.
The regulation specifies providing the name of the "Quality Improvement Organization (as set forth at sections 1152 through 1154 of the Social Security Act) that serves the area where the patient resides. These federally- and state-funded community-based services and organizations provide care for patients who are returning home or who want to avoid institutionalization entities, and are required by federal statute to help connect individuals to community services and supports."
The HHAs will need to include information about the state's Beneficiary and Family Centered Care Quality Improvement Organization (BFCC-QIO). This  BFCC-QIO fact sheet includes a coverage map with specific name and phone numbers. Please contact the applicable BFCC-QIO for additional details.
