Cardiovascular Health Improvement Initiative

Co-led by the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare and Medicaid Services (CMS), the national Millions Hearts® initiative brings together communities, health systems, non-profit organizations, federal agencies, and private-sector partners from across the country to fight heart disease and stroke. The HHQI National Campaign's Cardiovascular Health Improvement Initiative supports Million Hearts® by introducing cardiovascular best practices and data reports to the home health community.

HHQI's cardiovascular health improvement efforts focus on helping home health care professionals improve and measure progress on the ABCS of cardiovascular health: Aspirin as appropritate, Blood pressure control, Cholesterol management, and Smoking cessation.

Since Summer 2013, we have created new cardiovascular-focused Best Practice Intervention Packages and have begun providing relevant data resources to measure your progress toward improving preventive cardiovascular care and supporting the Million Hearts® goal of preventing 1 million heart attacks and strokes in the United States by 2022.
